Reporting an Incident
Is there someone in danger? Call 000 immediately.
There are two types of incidents.
1. A general Incident (When a person causes you harm or could have, when you hurt someone else, when you feel, someone is going to hurt you)
2. A reportable incident (A death, serious injury, abuse, neglect, sexual misconduct, unregulated use of restrictive practices)
We take both incidents seriously. If you as a participant, client or staff member / contractor feel unsafe, we want to hear from you as soon as possible so that we can address the incident.
Tell someone you trust about the incident and contact us on 0401 503 477 immediately.
You can also report your incident directly to us below.
